Get a disability certificate and UDID card
The Unique Disability ID (UDID) system issues a nationally valid disability certificate and UDID card, used for disability pensions, concessions, reservations, assistive devices and more.
Steps
- 1
Apply online on the Swavlamban Card (UDID) portal with personal details and a photo
- 2
Choose the disability type and the hospital/medical authority
- 3
Attend the medical assessment on the date given
- 4
Download the e-disability certificate and e-UDID card; the physical card is sent by post
Documents
- Recent photograph
- Aadhaar or other ID
- Address proof
- Any existing disability certificate or medical records
